Acquiring Birth Certificate, Marriage Certificate, Death Certificate, and CENOMAR at The PSA Now Made Easier

The days of waiting in long queues and processing is over. The process in getting  Birth Certificate, Marriage Certificate, Death Certificate, and Certificate of No Marriage (CENOMAR) has been made easy. You can even claim your document within hours. This is because of the computerized system that the Philippine Statistics Authority is presently using.
You can get request your documents by phone, online or going personally to their nearest office in your area.

For walk-in applicants, all you need to do is to follow these easy steps: 

 Step 1. Application
  Take an application form and fill-out the required information.

 Step 2. Screening
Proceed to the screeners' desk . This is where they check if the information you provided is readable and correctly done. They will mark it with "OK" if you are ready to proceed to the cashier.

 Step 3. Payment
Proceed to the cashier for the payment of your requested document. They will provide you with an official receipt indicating the release date of your document.

 At the cashier you need to pay the following amount depending on the documents you requested.

Step 4.  Releasing

If your request is not readily available due to some circumstances, you may need to present a request for processing which includes encoding, printing, sorting and releasing of the documents which may vary depending on the following cases:

Converted documents can be released on the same day.

Unconverted and annotated documents means they do not have the record yet on their database and you may need to wait for 10 days for your requested document to be released.

For the CENOMAR, you may need to wait for 4 working days for your document to be released.

There are not much requirements. All you need to do is the accomplished application form. Valid IDs are required for those who are requesting Birth Certificates. For those who are representing other people, you may need to present an authorization letter.

For busy people who doesn't have time to visit the PSA Offices personally, you may do it online by visiting e-census.gov.ph or through telephone by calling PSA Hotline at 737-1111.

REMINDER FROM PHILIPPINE CONSULATE OF JEDDAH: FAKE MARRIAGE CONTRACTS

This PUBLIC ADVISORY No 2016/55 dated October 24, 2016 was released and published by the Philippine Consulate General in Jeddah.

The Philippine Consulate of Jeddah reminds the public that the use of FAKE/FORGED documents submitted to Saudi Authorities is punishable by imprisonment and deportation. These include FAKE MARRIAGE CONTRACTS. Saudi Authorities have been requesting the Consulate General , through the Ministry of Foreign Affairs, for VERIFICATION of the authenticity of marriage contracts involving Filipino nationals.

The Consulate General only issues marriage contract for marriages between two Filipinos solemnized by the Consul General, Consul or Vice Consul within the Consulate. Consulate Officials do not solemnize a marriage between a Filipino and a foreign national.

For marriages solemnized in the Philippines, the Consulate General authenticates the marriage certificate issued by the Philippines Statistics Authority (PSA) and authenticated (Red Ribbon) by the Department of Foreign Affairs. 

The public is reminded that all transactions regarding official documents are done only inside the Consulate General premises, except during Consular Mobile Services outside Jeddah.

The Consulate General would like to further remind the public that FALSIFICATION of PUBLIC DOCUMENTS is a crime punishable under Philippine law.

Anyone who has information regarding individuals or groups falsifying documents purportedly issued by the Consulate General may report this matter directly to the Consulate Section Head, through 0555-2196-14 or email at consular@pcgjeddah.org

REQUIREMENTS AND PROCESSES FOR LATE REGISTRATION OF BIRTH, DEATH, AND MARRIAGE CERTIFICATE

Around 8 million Filipinos have not registered their names upon birth and therefore they cannot avail their civil and democratic rights such as secondary education and the right to vote.

According to Child-centered Plan International, a non-profit organization that two out of 10 children in 500 communities are not registered at birth in the Philippines.

Because of this, many children are denied of enrollment or can not graduate from high school because they have no birth certificates to prove their age or identity.

If you are one of those 8 million Filipinos who doesn't have a birth certificate or no record  in National Statistics Office (NSO) or in Local Civil Registry,  it is not too late. Also, we include in this post the process and requirements for late registration of death and marriage.

DELAYED REGISTRATION OF BIRTH

IF THE PERSON IS UNDER EIGHTEEN YEARS OF AGE

1. Complete the following requirements:

A. Four copies of Certificate of Live Birth (It should be duly accomplished and signed by the proper parties)

B. Accomplished the Affidavit for Delayed Registration at the back of Certificate of Live Birth by the father, mother, or guardian, declaring therein, among other things, the following: 

• name of a child
• date and place of birth
• the name of the father if the child is illegitimate and has been acknowledging by him
• if legitimate, the date and place of marriage of parents
• reason for not registering the birth within 30 days after the date of birth

If the party seeking the late registration of birth of an illegitimate child is not the mother, the party shall, in addition to the foregoing facts, declare in a sworn statement the recent whereabouts of the mother.

C. Any two of the following documentary evidence which may show the name of the child, date, and place of birth, and name of a mother (and name of a father if the child has been acknowledged)

• baptismal certificate
• school records such as nursery, kindergarten or preparatory
• income tax return of parent/s
• insurance policy
• medical records
• other, such as certification from the barangay chairman

D. An affidavit of two disinterested people who might have witnessed or known the birth of the child.

IF THE PERSON IS EIGHTEEN (18) YEARS OLD OR ABOVE


1. Complete the following requirements:

A. All the requirements stated above for the person who is under 18 years old;

B. Certificate of Marriage, if married

2. Bring the requirements and necessary documents and filed it into the Office of the Civil Registrar of the place where the birth is occurred.

3. The Civil Registrar will examine the application for delayed registration of birth whether it has been completely and correctly filled and if all the requirements are present or complied.

4. In the delayed registration of the birth of an alien, travel documents showing the origin and nationality of the parents shall be presented.

DELAYED REGISTRATION OF DEATH

Complete the following requirements:

A. Four copies of Certificate of Death, which must be accomplished correctly and completely

B. An Affidavit for delayed registration which shall be executed by the hospital or clinic administrator if the person died in a hospital, clinic or in a similar institution, or by the attendant at death if the person died elsewhere. 

In default of the hospital or clinic administrator or attendant at death, the affidavit shall be executed by any of the nearest relative of the deceased, or by any person having legal charge of the deceased when the latter was still alive;

C. The affidavit referred to shall state among other things,

• name of the deceased
• the facts of his death
• the date and place of burial or cremation
• circumstances why the death was not reported for registration within 30 days after death

D. Authenticated copy of the certificate of burial, cremation, or of other means of corpse disposal; and

E. Approval for registration by the health officer in the box provided in the Certificate of Death

DELAYED REGISTRATION OF MARRIAGE

In the delayed registration of marriage, the solemnizing officer or the person reporting or presenting the marriage certificate for registration shall be required to execute and file an affidavit in support thereof, stating;

• the exact place and date of marriage
• the facts, and circumstances surrounding the marriage, and
•  the reason or cause for the delay

If you completed all the requirements above depending on what kinds of documents you want to process, just submit them to your nearest Local Civil Registry Office. They will tell you how many days you have to wait to have your documents.

About the payment, according to National Statistics Office, this will be depending on the local government units (LGUs), because each one has its own Local Revenue Code or ordinances that governs the amount to be paid for those documents or penalty.


There are LGUs also that no longer impose payment in registering late for birth, marriage or death as part of their programs.

Going abroad is not an easy task.You need to prepare lots of documents and it takes more than your money to do it.You need to be ready and prepared just like you are going to war.
Getting the documents in the Philippines starting from a simple Residence Certificate(Cedula) takes a lot of patience.You need to go through endless queues,endure the hot weather,hot headed people and sometimes,even hunger.

One of these queues includes the authentication of documents at the DFA, Aseana. Although it will not take long and fortunately,you are going to do it in an air-conditioned room,you still need to go through some steps.


STEP 1.
Fill-out the Authentication Application Form that you can get from the Information section at the DFA,Aseana Authentication Division.Write legibly using a black inked pen.

Proceed to the Processing Window and present a valid ID upon submission of the documents.

STEP 3

Pay the corresponding fees to the Cashier.

For Expedite or Rush Processing(1 working day):  Php200/document

For Regular Processing (4 working days):               Php100/document

Step 4

Return duplicate copy of the receipt to the Processing Window.

STEP 5

Bring  machine-validated receipt and claim the Authenticated document on the indicated release date.

After claiming the Document,make sure that all entries are correct.In case that you found any error,you can proceed to the designated window to get it corrected.

The last step will be the filing-out of the

Authentication Client Feedback Form.This is something like an assessment based on your experience on how the Authentication Division of the DFA has served you.

Authenticated Documents are required  if you are going to use the said documents abroad.It certifies that the document is genuine.

Why do we need Authentication from the DFA?


There are many instances where DFA authenticated documents are being requested, for instance when applying for visa or immigration in some countries. 

A document that has been duly authenticated in the DFA shall  be deemed valid  in another country. 

"The Department of Foreign Affairs (DFA) performs its authentication function/services in the context of 1963 Vienna Convention on Consular Relations. Given the existence of a multi-state system, aforesaid Convention stipulates that, among other things, the various kind of documents sourced from particular sovereign states (e.g. public documents, private documents, and commercial documents) shall have validity or operative effect in another sovereign state's legal jurisdiction after subjected to an agreed upon authentication procedure.
The DFA through the Authentication Division-Office of Consular Affairs may authenticate an act, deed documents etc. executed or sourced within Philippine legal jurisdiction by way of certifying said act, as follows: a) executed before a local notary public officer authorized to execute such functions, b) testified to by a public seal, c) rendered public by the authority of a competent magistrate, d) certified as being a copy of a public register. When these acts, deeds or documents have been duly authenticated by the Department, the receiving embassies or consulates or any other foreign legal entities are, in effect assured that aforesaid documents are in order or have been legalized in accordance with proper procedure. Foreign Embassies, Consulates and foreign legal entities may or may not require the authentication of documents sourced from the Philippines for use within their respective jurisdictions. However, under rules of reciprocity in international relations, sovereign states normally require authentication of foreign documents to be used.
The DFA authentication attest to the genuineness of the signature appearing on the attached certification for the purpose of identifying a specific document and giving credence to the official acts of the notary public or certifying officer thereon for use, if and when required, as instrument of evidence in foreign countries. As such, the DFA certification/authentication does not, per se, authenticate the validity/efficacy/enforceability of the basic document itself. However, the Authentication Officer is impressed with duties calling for carefulness and faithfulness whereby he must inform himself of the facts to which he intends to certify and avoid participation in illegal enterprises.

 What type of documents need authentication?
The following documents can be authenticated.

1. NBI Clearance

2. NSO issued documents in security paper including CENOMAR

3. Barangay Certification w/ Mayor Certification*

4. Police Clearance*

5. Affidavits of Employment, Trainings/Seminars,  and Baptism/Confirmation w/ Regional Trial Court (RTC) Certification*

6. Other Notarized Documents(SPA, Affidavit of Support and Guarantee, Consent, etc.) with RTC Certification*

7. Medical Certifications authenticated by DOH

8. PRC Documents**

9. LTO License Certification(issued by LTO main office only)

10. Foreign Documents authenticated by the Philippine /Consulate or documents authenticated by Foreign Embassies/Consulates based in the Philippines.

11. School Records (TOR, Diploma, Form 137, etc) w/ CHED, TESDA, or DEP-Ed Certificate of Authentication and Verification (CAV)**

12. School Records with CAV issued by State Colleges and Universities**

In Addition, regardless of affinity, an applicant may send an authorized representative to apply on his behalf. Authorized Representative must have a valid ID and has to present a signed authorization letter or SPA from the applicant, a photocopy of applicant's ID bearing his signature, and a photocopy of the authorized representative's ID.

According to the Authentication Rules and Regulations (ARR) of the Department of Foreign Affairs, beginning 21 March 2016, an authorized representative is only allowed to process and claim documents on behalf of the owner provided that he/she presents the following:i. His/her own government issued identification card and a photocopy thereof;

ii. A Special Power of Attorney (SPA) from the owner of the document; and

iii. A copy of the applicant's passport or a copy of any identification card bearing the signature of the applicant.

However, a representative is limited to file and/or claim documents of only 10 applicants per week. Further, the SPA must indicate the name of the representative.

Note:

1. * Issued within Regions I, II, III and CAR.

2. **Issued within the NCR, Regions I, II, III and CAR.

3. In case the document needs further verification, the applicant is advised to file at Authentication Division, Aseana      Building, corner Bradco Ave., Macapagal Blvd. Parañaque City.

4. Liaison Officers of Recruitment Agencies are not allowed to transact authentication services at any Regional Consular Office (RCO).

WARNING!

DO NOT ATTEMPT TO SUBMIT FAKE DOCUMENTS!

Any fake or tampered documents submitted for authentication are subject to outright seizure upon proper determination by the DFA Authentication Officer. holder of said documents, as well as fixers are liable to prosecution.

ADVISORY

Unclaimed Documents will be disposed  after three months from Date of Release.
