WHO CAN AVAIL UMID?
⦁ For those government employees you could apply in GSIS Enrollment Center; while

HOW CAN I GET MY UMID?
FOR GSIS MEMBERS
⦁ Fill up the enrollment form for UMID or download the following:
⦁ Present your e-card Plus or 2 valid ID's
⦁ The staff of GSIS will validate your information written in your enrollment form and your requirements
⦁ Once it was done, you will take an ID picture, fingerprints, and signature
⦁ You will receive an email; when you will get your ID, most of the time takes 1 to 2 weeks
⦁ Note: Even if you have an eCard, you are obliged to get the UMID and there are different enrollment form for that.

FOR SSS MEMBERS
⦁ Fill up the E-6 application form or download form here
⦁ Present 2 valid IDs (original and photocopies)
⦁ SSS staff will verify if you are an active member and eligible to get a UMID card.
⦁ Once it was done, you will take an ID picture, fingerprints, and signature
⦁ Wait for the 2 weeks to get your card
You can present one (1) primary valid ID's such as;
⦁ Passport
⦁ Professional Regulation Commission Card (PRC Card)
⦁ Driver's License
⦁ Seaman's Book
If in any case, you don't have the primary ID's, you can submit two of the following documents:
