Requirements
•Completely filled-out bank forms:
•Application and Agreement for Deposit Account (AADA) Form
•Customer Information and Specimen Signature Cards (CISSC)
•For OFWs: New clients will be asked to declare the employer and country of employment.
•For OFW Beneficiaries: Indicate the name, relationship, contact details, country where OFW remitter works and employer under "Endorsed/Referred by" portion of the CISSC form.
•Presentation of at least two valid IDs bearing your photo and signature
