UK: LOST PASSPORT REQUIREMENTS, PROCESS AND FEES

UNITED KINGDOM

1. Two (2) completed passport application forms. DOWNLOAD FORM HERE
2. Two (2) completed Affidavit of Lost Passport
3. Original and (1) photocopy of PSA-issued Birth Certificate/Report of Birth
4. Original and (1) photocopy of PSA-issued Marriage Certificate/Report of Marriage, for married female applicant
5. Photocopy of the data page of the lost passport, if available
6. Photocopy of all pages of the damaged passport

Note:   There is a 14-day waiting time before the replacement application may be given due course

           

On filing the application to replace a lost/damaged passport, applicant shall be made to pay GBP5, for the verification of passport issuance and notification of all PHL Embassies/Consulates of the lost passport.  Applicant shall be requested to return a week after filing, for an interview.  If there are no adverse findings at the interview, the applicant will be allowed to proceed with the passport application.
