• 1. Duly accomplished Passport Application Form (Annex A)Original (and photocopy of )

• 2. Duly Authenticated Birth Certificate and

• 3. Original (and photocopy of) Duly Authenticated Marriage Contract, for married women 
• 
  
• 4. Affidavit of Loss, with fee of €22.50 
• 
  
• 5. Police Report / Denuncia 
• 
  
• 6. Application Fee of €135.00 (for lost ePassport) or €81.00 (for lost MRRP or MRP) 
• 
  
• 7. Photocopy of lost passport, if available 

 Other documents as may be required, showing photo and signature of applicant,  like the following:

• Voter’s ID, Baptismal Certificate, PRC IDF, Driver’s License, Seaman’s BookTranscript of Records or DiplomaGovernment Service Record / GSIS / SSS IDsPermesso or Carta di Soggiorno (if documented)
