Our body is our most precious possession, that is why it is important that we have to take good care of it. But there are times that even how hard we take good care of ourselves, we get sick and ill that needs a long process of medication.

We all know that hospital bills are not cheap nowadays, especially if we are receiving health care from private hospitals even if we are in government hospitals, the cost of medicines is still pricey.

Aside from the PhilHealth and Department of Social Welfare and Development (DSWD), don't you know that you can also avail of medical assistance from Philippines Charity Sweepstakes Office (PCSO)?







This is an Individual Medical Assistance Program (IMAP) that aims to provide timely and responsive financial assistance to individuals with health-related needs.

WHO CAN AVAIL THE IMAP?

All Filipino who request for assistance, either personally or through a representative, on a health-related concern, including:
  • Those confined in any health facility
  • Those receiving health care management as outpatients
  • Those seeking management in foreign countries provided that no health facility within the Philippines is capable of providing such procedure





WHAT ARE THE HEALTH SERVICES COVERAGE?


1. Confinement – Currently admitted or discharged with promissory note duly acknowledged by the health facility



2. Dialysis – Such as, but not limited to Hemodialysis and Peritoneal (Continuous Ambulatory Peritoneal Dialysis [CAPD])


3. Cancer Treatment – Such as, but not limited to Chemotherapy, Oral (Hormonal), Intravenous, Radiation Therapy, External Beam Radiation Therapy, Brachytherapy, Radioactive Iodine (RAI), and Stereotactic Radiosurgery

4. Medicines, such as, but not limited to Anti-rabies, Psychiatric, Antibiotics, and Factor 8 and 9

5. Surgical supplies

6. Implant, such as, but not limited to bone and cochlear 

7. Laboratory/Diagnostic Procedures 

8. Devices, such as, but not limited to medical devices and assistive devices 

9. Non-and Minimally Invasive Procedures, such as, but not limited to Extracorporeal Shock Wave Lithotripsy, Laparoscopic surgery, and Endoscopic procedures 

10. Transplant procedures, such as, but not limited to kidney transplant packages and liver transplant packages 

11. Cardio Procedures 

12. Rehabilitative Therapy (Physical, Occupational / Speech Therapy)


WHAT ARE THE DOCUMENTARY REQUIREMENTS TO AVAIL?

GENERAL REQUIREMENTS

  • Duly accomplished PCSO IMAP Application Form
  • Original/Certified True Copy of Updated Clinical Abstract duly signed by the attending physician with License Number
  • Valid Identification Card of the Patient and Representative, including any government issued ID, company ID, or student ID
  • Authorization Letter from a patient in cases where there is no immediate relative available for interview
SPECIFIC REQUIREMENTS

A. For confinement, you need to prepare the following:

  • Original copy of the Statement of Account/Hospital Bill duly signed by the Billing Officer/Clerk Supervisor
  • Endorsement from the Medical Social Services of the health facility (for charity/service patients)
  • Police Report (for medico-legal cases)
  • Duly executed Promissory Note, if discharged
B. For dialysis (procedure, medicine and peritoneal dialysis) you need the following;
  • Endorsement/Certificate of Acceptance of PCSO Guarantee Letter (GL) (Shall no longer be required in the case of facilities where the PCSO Desk has been established)
  • Official Quotation from service provider
  • Relevant laboratory result
  • Certification from Dialysis Center
  • Prescription, for medicine and peritoneal dialysis
  • Certification of PhilHealth membership
C. For medicines and chemotherapy, you need the following;

  • Relevant laboratory result
  • Original and latest prescription duly signed by physician with license number
  • Photocopy of Histopath/Biopsy report (for chemotherapy)
  • Progress note from the doctor/oncologist (for ongoing chemotherapy)
  • Prescription of medicines or chemo drugs
  • Treatment protocol (additional requirement)

FOR CHEMOTHERAPY REQUEST

A. For radiation therapy, you need the following;

  • Endorsement/Certificate of Acceptance of PCSO Guarantee Letter (GL) - (Shall no longer be required in the case of facilities where the PCSO Desk has been established)
  • Official quotation from service provider with breakdown of expenses and deduction from PHIC

B. For Surgical Supplies, prepare the following;

  • Official quotation with breakdown of expenses
  • List of operating room needs/supplies
C. For Implant (Bone/Cochlear), you need the following requirements;

  • Official sealed quotations from three (3) suppliers
  • Copy of the physician’s order with implant specification
  • Police Report (for medico-legal cases)
  • Date and schedule of operation
  • Copy of the result of relevant procedure i.e. X-ray, CT Scan, and MRI
D. For Laboratory and diagnostic procedures, you need to prepared the following;

  • Official Quotation from service provider with breakdown of expenses
  • Laboratory request duly signed by physician with license number
E. For Medical devises, (Pacemaker/Septal Occluder/PCI), prepare the following;

  • Official sealed quotations from three (3) suppliers
  • Copy of result of procedure, i.e. 2D echo, ECG, and Angiogram
F. For Hearing Aide, requirements are;

  • Official sealed quotations from three (3) suppliers
  • Audiological Evaluation Report signed by a licensed Audiologist
G. For Wheelchair, you need the following;
  • Official sealed quotations from TahanangWalangHagdan
  • One (1) whole body picture
H. For prosthesis, you need the following requirement;

  • Official sealed quotations from three (3) suppliers
I. For Pumonary Apparatus, you need;

  • Official quotation from service provider
J. For Non and minimally invasive procedures

  • Endorsement/Certificate of Acceptance of PCSO Guarantee Letter (GL)(Shall no longer be required in the case of facilities where the PCSO Desk has been established)
  • Relevant laboratory result
  • Costing of procedures
K. For Transplant, you need the following;
  • Relevant laboratory result
  • Cross-matching result
  • Certification of patient’s inclusion in transplant program from authorized representative of National Kidney and Transplant Institute (NKTI), if applicable
  • Relevant Philhealth tracking number certification
  • Costing and breakdown of transplant expenses
L. For Rehabilitative therapy (Occupational, Physical and Speech Therapy
  • Official quotation from service provider with breakdown of expenses

To process and fast-track your assistance from PCSO, make sure, your requirements are complete before you submit it to PCSO Office.
But the good thing is PSCO has established its help desk known as At Source Processing or ASAP Desk and Enrollment System for Chemotherapy and Dialysis in 44 hospitals nationwide. This ASAP Desk aims to facilitate accessibility in the filing and processing of a request for IMAP for indigent patients confined in the affiliated hospitals.

It means if a hospital has a PSCO ASAP Desk, patients need not go to PCSO office to file their request for assistance.
They can immediately file their request and submit the necessary requirements at the PCSO Help Desk.
